哈佛醫師之路

Havard Medical School -- 田中真由美

前天長官送我這本書,附上的紙條是要謝謝我幫他公子的老師尊翁看診,但實際的用意應該是要叫我好好反省。我可以感受到長官的用心良苦,該說謝謝的是我。

書中提到:身為醫師必須採取徹底的低姿態。與病患的接觸方式,對疾病的治療有莫大的影響。

雅各書 1 章19 節後:但你們各人要快快的聽、慢慢的說,慢慢的動怒。

醫生所說的每一句話,對病患與家屬可能都會有很大的影響;特別是對處在困惑與無助狀態下的病人。

看完這本書,我必須說,心底升起極大的惶恐,不再覺得自己適合這個行業。因為我思緒很急,反應過快,工作滿檔了,還不得不逼迫自己再多作一點;每次看到薪水條,都覺得是用命換來的。

我對自己,週遭的同事,包括我的病患,都太急了。我得好好想出取得工作負擔與醫療態度平衡的對策。

看過這本書
不盡然同意其中的許多觀點
而且醫療環境相差太多
台灣許多醫師的低姿態已經到卑微討好甚至是放棄自己專業的程度
因為不如此在這競爭的醫療環境中很難闖出一番天地
但是自己的專業呢
ㄧ個醫師如果一味討好病人(當然這跟書中所低姿態意義不同)而不堅持專業那他還可以稱之為醫師嗎
在台灣要當一個好醫師是很辛苦的
當一個病人不遵醫囑時醫師仍然是要維持低姿態嗎
當然這也可以說是病人自己的選擇吧
但是醫師有選擇嗎

當然有-醫生可以有的選擇,多過病人不知多少,病人生病才沒得選擇。

I've got your point.
For example, a patient with a non-clearing vitreous hemorrahge and multiple systemic diseases always came to my clinic with a non-stop complaint of blurring.
In the beginning, I'd try my best to be polite to suggest him quit smoking and tell him where he can get help.
But somehow, he just couldn't make up his mind to quit.
OK! I could accept and respect it's his choice if he didn't want to do something for his eyes.
But could he stop complaining and asking me to improve his vision ?
I was frustrated by this kind of patients before.
Now I've decided to let go, though it's hard.

My doc friends, remember that little area called psychology?

If you suggest to a patient, go to see a spiritual adviser(psychiatrist to you and me), your eyes(or xyz) will get better a lot faster. Hopefully these patients will get some real help to ease their anxiety and many of them do need meds... some happy pills maybe. Ha ha.

So, it's not your job to MAKE them feel better, just your responsibility to point them in the right way to get help.
